Please make sure to have all necessary original paperwork and fill out all registration forms, then call us to set an appointment to finalize registration at 305-931-4881.
Requirements: Originals forms only
1. Birth certificate
2. Parents/Guardians ID (Driver’s License)
3. 2 proof of address (light bill and lease agreement or house deed in the name of parent/guardian)
4. Physical examination (DH3040) and FL Immunizations certificate (DH680)
5. Fill in blue ink the registration forms below. If not able to print forms, call us to make appointment to pick up forms)
Registration 9 am to 2 pm Monday to Friday by appointment.
